DO YOU HAVE MEDICAL RECORDS THAT PROVE YOUR QUALIFYING CONDITION?
YES, I have medical records that proves my qualifying condition, or I can get them by my visit time.
I do not have medical records that prove my qualifying condition, and I would like you to help me get them.
If you do not have copies of your medical records and would find it difficult to go to your doctors office and pick them up before your visit, you can request them with this form. Please note, doctors offices can be slow to send records to other doctors offices, and this may delay your visit time. It is usually quicker to go to the office and get them yourself. However, if you can wait 1-3 weeks for your appointment, please fill this form and we will contact you as soon as we get the records. Medical Records
Will I qualify?
If you have proof of your qualifying condition, you will qualify. At your certification appointment, our physicians will talk with you about the medical condition that qualifies you for the Florida Medical Marijuana Program. Rarely, the doctor may request more documents if your records are not enough to meet the state requirements. After review and approval, the doctor will produce the documents needed for the state application.
Is it safe?
Medical marijuana is one of the safest medications available. Period. There has not been a single death recorded from an overdose of cannabis. Most of the few significant negative events with cannabis involve untreated or latent schizophrenia. Chronic use of cannabis causes no organ damage, and it has been shown that, in adults, the cognitive effects sometimes associated with chronic cannabis use dissipate when cannabis intake is discontinued. This is in contrast to even acetaminophen which causes thousands of hospitalisations and kills hundreds of americans every year. (Statistics)
Will the Certification Center be discreet?
We always seek to be protective of your reputation. The office you walk into doesn’t have MARIJUANA written all over it. There is no GIANT MARIJUANA LEAF all over the door. You walk into a medical office and visit with a doctor. Our Receipts and Records Request forms come from a Doctor’s Office, NOT from a Medical Marijuana Office (AND our receipt is a valid medical expense for tax purposes.) It is your decision whether your doctor or the neighbors know anything about your business. We maintain the full confidentiality required by law. Other than submitting your application, we never report or submit ANYTHING to any agency. We are the most discreet Florida Medical Marijuana Card services you can find.
What is the process?
In most cases, you must have a prior diagnosis with one of the conditions above. This means that you must have a chart note from a healthcare provider proving your diagnosis, and mentioning the treatment that was offered.
You can submit a records request through our online records form. It will be sent to your doctor automatically, and hopefully, they will fax us the records.
In some cases, like obvious physical conditions, the doctor can diagnose in the visit, and records are not necessary, but the visit may require more time, and an additional charge may apply.
After the visit, we submit your application to Florida Health. It takes 30 days to receive the approval email. After that, you call us and we submit an order for cannabis products based on your condition and preferences. The maximum time for a certification is 210 days, so you would have to renew your card after that time.
Will Medical Marijuana help me?

In some cases it is curative, but mose it reduces symptoms and improves quality of life.
Because it is so safe, it is worth trying if you have an approved condition.
